I am a new employee, when am I eligible for benefits?
Full Time Employees are eligible for benefits on the 1st of the month after 60 days of employment. For example, you are hired on June 15th, you will complete 60 days on August 15th and will be eligible for benefits beginning September 1st.
I am a new Employee; how do I enroll in medical benefits?
Upon hire; you are given access to enroll in benefits online through the Dayforce portal. You can begin enrolling in benefits on your first day of hire and have 60 days to complete your enrollment. You must be fully enrolled in benefits before the first of the month that your benefits are scheduled to begin. If you do not enroll prior to this timeframe, you will not be allowed to enroll and will have to wait for open enrollment or a qualified live event. Information on benefits available and the details for each policy are found on this site.
I was a Part Time Employee that just moved to a Full-Time position, what benefits can I enroll in?
Congratulations on your promotion to a Full-Time position! You are now eligible to enroll in Medical, Dental, Vision and Prescription insurance through Regence Group Administrators (RGA) as well as Flexible Spending Account (FSA) through Navia. Please log on to your employee portal on Dayforce to complete this ‘work event’ change and enroll in those benefits. You have 30 days from the date you became Full Time to complete this enrollment.
I haven’t received my medical card yet, what do I do?
If you know your member ID number, you can log in to the insurance website www.accessrga.com using your information and print a temporary card. Alternatively, you can call RGA Customer Service at 866-738-3924 to ensure they have your correct address, and you can request another card at that time. To update your address, please do so through the Dayforce portal. Please contact the HR Department at benefits@oakh.com or call us at 206-865-0167 for additional assistance.
I just got married and/or had a new baby (or adopted), how do I add them to my coverage?
When you have a “life event” you have 61 days from the event to add any new dependents on to your medical plan. You will be able to log into your Dayforce account and enroll your new baby, spouse, or stepchildren to your plan electronically. You’ll be required to upload back up documentation as well. Back up documentation typically includes a marriage certificate and/or birth certificate if adding children..
Can I add my girl/boyfriend, Domestic Partner, or their children to my plan?
Unfortunately, no. You would have to be legally married to your spouse to add your spouse and/or stepchildren to any benefit.
How can I find out what benefits I am currently enrolled in?
Please log on to your Dayforce portal; there you will be able to view all benefits you are enrolled in and see the cost you are being deducted for.
I have additional questions about my enrollments, who can I contact?

I need to make changes to my enrollments, or add/remove dependent(s); how can I do that?
In some instances, if you experience a qualified life event, you may be able to remove, add or update dependents from your plans outside of annual open enrollment. Please contact benefits@oakh.com or call 206-865-0167 and ask for an HR Team Member to assist you in this process.
How much do I pay for my coverage?
Premiums are based off your gross wages and the type of plan you selected. Premiums are paid the month the coverage is effective and are deducted on the 7th and 22nd check each month. Premiums are also deducted for RGA from any PTO or bonuses paid out. A $100.00 per check Spousal Surcharge will be applied to any Spouse enrolled on the plan that has coverage through their employer. Employees can submit documentation to stop this surcharge at any time. Note: Family coverage is defined as Employee, Spouse and Child(ren).
My spouse doesn’t work; do I still have to pay the Spousal Surcharge to keep them on my plan?
No, there is no surcharge paid since the spouse isn’t offered an employer group plan.
My spouse receives SSI, Medicaid or Medicare, can the Spousal Surcharge be waived?
Yes, as these are not group plans through your spouse’s employer.
My spouse also works for Oak Harbor; do I still have to pay the Spousal Surcharge?
No, the surcharge is waived if both employee and spouse work for Oak Harbor.
My spouse works part time; can I waive the Spousal Surcharge?
If your spouse is not eligible for group coverage through their employer, you can waive the surcharge.
Who is our dental coverage through?
Your dental coverage is with RGA (Regence Group Administrators). This is the same company that manages your medical claims. The card you were issued is what you need to provide to your dental office.
Who is our vision coverage through?
Your vision coverage is with RGA (Regence Group Administrators). This is the same company that manages your medical and dental claims. The card you were issued is what you need to provide to your eye doctor’s office.
I would like to decline medical/dental/vision coverage, what do I do?
There is a ‘waive’ option in the benefit section of the online Dayforce portal. By clicking on that option, you will waive the coverage for each benefit you wish to decline. You can waive benefits during your initial hiring period, during a qualified life event or open enrollment.
I am at my provider’s office or pharmacy, and they are saying I do not have coverage?
First, confirm with the person you are speaking with that they are calling the eligibility phone number on the back of your RGA medical card. Some offices have a speed dial phone number set up and it is usually the incorrect phone number. If they call the number on the back of your benefit card and are still being told you do not have coverage, you will need to call the Customer Service number for RGA 866-738-3924 or the Benefit Resource Center 866-468-7272.
How long can I keep my children on my insurance?
Through RGA (medical plan), children can be covered until age 26, regardless of student or marital status. They will be automatically dropped the end of the month they turn 26 yrs. of age. If you have an adult, disabled child that you care for, coverage can continue past the age of 26, but special documentation is needed. If you need to request this extended coverage, please contact the HR Department at benefits@oakh.com.
For all Unum plans; children can be covered until age 26, regardless of student or marital status. NOTE: they are not automatically dropped from your plan, you are responsible for dropping your child(ren)’s coverage when they age out.
I received a bill from my medical provider, and I do not believe I should have? Or the bill is incorrect?
The Benefit Resource Center 866-468-7272 can assist. They will work on your behalf with the Provider’s office and RGA to either correct the bill or clarify whether you were billed correctly.
How can I find out whether a specific item (medical procedure, prescription, etc...) will be covered by insurance?

Does Oak Harbor offer an Employee Assistance Program?

Does Oak Harbor offer a Wellness Benefit?
Yes, through the end of 2026, Employees and Spouses enrolled on the RGA medical plan, are eligible for an annual Wellness Benefit of $50 each. If you, and your Spouse complete a physical that includes full blood panel testing, you are eligible to receive this benefit. Simply print off the Proof of Annual Physical Form from this site, or you can find it on the Dayforce Benefit Hub page and give it to your providers for a signature. Then, upload and submit in Dayforce. Wellness benefits are paid on the following paycheck after they are received.
What is Wellworks and how does it affect me?
Wellworks For You is a program designed to build healthy, preventive habits by going to a medical provider and completing a physical & bloodwork screening annually. Completing these steps and filling out a Tobacco Attestation Form annually will keep your healthcare premiums from increasing. Full Time Employees and their Spouses, if enrolled on an OHFL Medical Plan (Plan 1, Plan 2 or Plan 3) are eligible. Those enrolled in only vision and/or dental plans are excluded.
What happens if me, or my spouse, don’t get an annual physical or submit requirements for Wellworks?
Employees, and enrolled Spouses, that do not participate, may see an increase in their Wellness Rate (the amount you pay for the RGA Medical Plan) in 2027. BOTH Employee AND Spouse must participate to avoid a Wellness Rate increase in 2027.
What happens to my benefits should I leave Oak Harbor?
If applicable, COBRA paperwork will be sent to you for any RGA plan you were enrolled in. This will come direct from RGA mailed to the last known address on your Dayforce profile. COBRA will allow you to continue any medical benefits and pay RGA direct.
For any Unum benefits, some of those policies are portable. You will receive information from the HR Department on how to port or convert applicable policies to pay direct to Unum.
If you had a Health Savings Account (HSA), you will still have access to any funds contributed by yourself or Oak Harbor upon your departure. They do not expire.
If you had a Flexible Spending Account (FSA), you will not be able to use your card for expenses past your departure date with Oak Harbor.
Am I required to sign up for mail order prescriptions?
Our plan requires members to use the Optum Home Delivery program for long-term maintenance medications. Optum Rx can provide you up to a 3-month supply, depending on the prescription, and you can even enroll in automatic refills. If you have some special circumstances in which mail order is not the best option for you, please contact the Benefit Resource Center 866-468-7272 or the HR Department at benefits@oakh.com or call HR at 206-865-0167 to see if you could be allowed to continue using your retail pharmacy.
How do I sign up for the Optum Home Delivery program?
You can ask your providers office to send the electronic prescription to Optum Rx. Or you can log into the Optum Rx portal at www.optumrx.com, through the Optum Rx app on your phone or call the pharmacy number on the back of your RGA medical card.
Once I signed up for home delivery of medications, how long does it take?
Medications should arrive within 5 business days after Optum Rx receives and completes your order. You can track status by logging into your account at www.optumrx.com, through the Optum Rx app on your phone.
Why is my prescription not covered?
If you are told that a prescription is not covered, it is recommended you call the Benefit Resource Center 866-468-7272 to get their assistance why it is not covered. It is possible that this particular medicine is not covered, but an alternative may be covered. If this is a maintenance medication, taken on a regular basis, our plan requires Employees to use the mail order system after filling the prescription 3 times at a store-front pharmacy.

Why a flat amount instead of included in the RGA employee contribution percentage?
When the company stepped back and looked at how we could change the plan. We could have just upped the costs for everyone. As example, we up the percent to 6% for everyone and no spousal surcharge. That would leave a one income family paying 6% for the coverage, just the same as a dual income family where the spouse has the option to get insurance at their own company but chooses to use Oak Harbor’s insurance.
Spouse does not work and is on my plan, how much do I pay?
There is no surcharge paid since the spouse doesn’t have a plan. You will only pay the additional percentage based on the plan selected.
If my spouse finds employment how does Oak Harbor track that?
We have no way of tracking that, we will rely on the honor system and employees being honest about their spouse’s employment and when a surcharge should be paid.
Can the spousal surcharge be split between the two pay periods each month?
Yes, starting in 2026, $100 will be deducted every regular paycheck.
Are there arrears if you are on leave?
If you are on FMLA, you are not required to pay the surcharge. If you are not on FMLA and miss a payment, arrears will calculate and will be deducted from future checks upon return to work.
What if it’s more expensive for my spouse to get insurance through their work?
The employee would still be charged $200 each month as long as their Spouse has other coverage available and they are on the OHFL plan.
If employee’s spouse is on federal medical and requires dual coverage, can it be waived?
Yes, it can be waived because the Spouse is not employed.
If my spouse also works for Oak Harbor, do I still have to pay the surcharge?
No, the surcharge is waived if both employee and spouse work for Oak Harbor.
If my spouse works, but only part time, can I waive the surcharge?
Yes, if your spouse is not eligible for group coverage as a part time employee, you can waive the surcharge on your plan.
Dependents, do they need to show proof of student for life insurance?
Not unless the policy needs to be used and asked for by Unum.
How long is my child(ren) eligible to keep Unum benefits?
Children are covered on all Unum plans up to age 26, regardless of marital or student status.
Does Unum automatically drop my child(ren) when they age out?
No, Employees are responsible for dropping ineligible child(ren) when they age out.
Will the HSA roll over?
Yes, it rolls over each year….different than the FSA that does not.
HSA is Pre-Tax?
Yes.
Can HSA help cover deductibles?
Yes.
Is there a limit on HSA contributions?
The 2026 HSA limits are $4,400 for individual coverage and $8,750 for family coverage. This must consider the OHFL contributions of $800 and $1600. So, the total an employee can contribute would be $3,600 for individual and $7,150 for family. Individuals over age 55 can contribute an additional $1,000.
Can you explain HSA?
You can only enroll if on the QHDHP plan #3. The HSA account is opened for the employee automatically so Oak Harbor can contribute their portion. The Employee can use it to help pay for the deductible, or other medical expenses. Employees can also contribute if they want. We have additional information that will be on the benefits website by Nov. 1.
If I am on HSA can I also contribute to FSA?
Yes, FSA has a Limited Purpose Account you can sign up for.
There are 3 different FSAs and the limited is the only one that works with the HSA.
Will it be the same Blue Cross Blue Shield/RGA plan?
Yes, however there are some deductible or out of pocket changes. Please refer to the 2026 Benefit Guide for details.
What are the benefits of a higher deductible plan?
Employees without a lot of medical expenses/do not foresee lots of medical expenses in the coming year; or do not have daycare expenses may be a better fit for this plan.
If my child is enrolled in college and is under 26 yrs. old, are they still covered under RGA, if transitioning to graduate school in the fall?
All children are covered on RGA up to age 26, regardless of student status.
My online 401k account (NWPS website) does not reflect the correct vesting percentage, who do I call?
Please contact NWPS at 888-700-0808 to ensure that your account correctly reflects your vesting percentage.
How do I make a change to my 401k contributions?
401k contribution changes can be made at any time during the year and can be done online through the Dayforce portal using the 401k Enrollment & Change Form. Changes will take effect the following pay period.

How do I sign up for direct deposit?
Please log on to your employee portal in Dayforce to sign up for direct deposit. If you have questions, please email payroll.dept@oakh.com or call 206-865-0167.
I don’t have a checking account, but would like to receive my paychecks electronically, do I have any options?
Yes, we are linked with Dayforce Comdata pay cards that act as debit card and can be used anywhere. You can request this card online through Dayforce by using the “Request Payroll Card from Manager” form. Please contact payroll at payroll.dept@oakh.com or 206-865-0167 if you have any questions.
I have questions about my account or investment options, who can I contact?

I don’t think I’m being deducted correctly for my benefits, what can I do? It’s very important to review your paystubs every payroll to make sure you are being deducted correctly. If you find an error, please email benefits@oakh.com or call 206-865-0167.
I need to be off work for more than 3 days due to a medical reason, or to care for a family member, who do I report this to and how are my benefits affected?
Please contact your Manager as soon as you know you will be off of work for more than 3 days for any reason. Your Manager will contact the Human Resources Department to begin the process of your request for leave. You can also call the Human Resources Department at 800-858-8815 and ask to speak with a member of the team regarding the types of leave available.
If I qualify for FMLA, does that pay for my wages while I am off work?
FMLA is a law that protects your position and pay for up to 12 weeks every 12 months. There are specific requirements that must be met to be approved for FMLA. If you are approved for FMLA, you may be required to use any paid time off benefits you may have in your bank. If you work in a state that offers State Disability, you will need to contact your state’s disability department to apply for paid benefits.
What if I don’t qualify for FMLA and still need to be off work?
If you do not qualify for FMLA; you may be eligible for additional medical, personal leave or protected state leave. Please contact your Manager or the HR Department for more information.

If I don’t use my Flexible Spending Account money, can I roll it over to the next calendar year?
Flexible Spending Account money must be used each calendar year, this is a “use it or lose it” plan. Any funds not used during the year will not carry over into the next year. It is important to carefully select the amount you sign up for each year, know what exactly is covered, and keep proper documentation of your receipts for proof that is required by Navia Benefits.
Does the company offer other supplemental insurance?
Yes, the company offers a Supplemental Life & AD&D, Short Term Disability, Accident, Hospital and Critical Care insurance through UNUM. All policies, except Short Term Disability, are available for yourself, spouse, and children.
I am a new Employee, when can I sign up for Unum supplemental benefits
Supplemental Life, AD&D, Accident, Hospital, Critical Illness and Short-Term Disability benefits through UNUM are available to new Employees when they are hired. You can begin enrolling in benefits on your first day of hire, and will have 60 days to complete your enrollment. I heard the company pays for Life Insurance for Employees, is that correct?
Yes, that is correct. Each Full Time Employee has a $15,000 life insurance policy paid for by Oak Harbor. This is a free policy, at no cost to you. We just need your beneficiary information on file, this can be updated anytime through the Dayforce portal.
